Education Requirements

Bachelor's Degree in Education or Human Service related field preferred. Two years' experience in supervision and five years' experience in a program for developmentally disabled adults.

Responsibilities

Essential Duties and Responsibilities:
Providing clients with the highest quality of support and services.

1. Responsibility for the overall administration of assigned Habilitation Programs
2. Maintain conformance with CARF and all applicable standards, rules, regulations, laws and administrative codes.
3. Train and supervise Direct Support Professionals in assigned Habilitation programs.
A. Ensure staff follows prescribed training and best practices regarding service delivery in:
• Implementing approved Individual Support Plans
• Therapeutic and behavioral interventions
• Assistance with feeding and other personal care tasks
• Dispensing medications
• Lifting and transferring techniques
B. Work with team members to implement daily activities for clients including:
• Small group activities
• Individualized therapeutic activities
• Facility based activities
• Activities offered elsewhere in the community
4. Monitor Cost Center budget information and make budget recommendations to the Director of Day Services in preparation for fiscal year budgeting and planning.
5. Ensure the use of state-of-the-art curriculums for Direct Support Professionals. Assist Direct Support Professionals as needed in the use of curriculums and functional assessments as needed. Make recommendations to the Director of Day Services for the acquisition of training materials as needed.
6. Ensure that goals and objectives for Day Services clients are being followed. Ensure that program services are being delivered on an ongoing basis, thus ensuring active treatment to those clients within that program area
7. Provide direct instructional time as Direct Support Professional to clients to ensure all client-training goals are implemented according to the ISP.
8. Monitor client behaviors and communicate incidents of special behavior, safety/health concerns to the Director of Day Services and/or QDDP. Ensure all reportable incidents are reported as required by the State incident reporting process.
9. Complete monthly Medicaid Waiver reports, Progress Notes and other documentation as required.
10. Write individual program goals and objectives with input from QDDP staff and other personnel as needed.
11. Participate in clients’ quarterly progress meetings as scheduled.
12. Prepare Annual Case Conference recommendations for goals and objectives for Day Services and participate in the meeting with interdisciplinary team.
13. Manage the daily operations of the Habilitation Program including scheduling of staff, program development, and maintaining sufficient personnel to meet individual client goals.
14. Ensure all Direct Support Professionals maintain training requirements.
15. Participate as a member of the Day Service group and attend weekly staff meetings.
16. Participate as a member of the Program Team and attend monthly meetings.
17. Ensure compliance with all organizational policies including but not limited to Governance, Personnel Policy and other approved policies and procedures.
18. Assist the Director of Day Services on the Annual Plan as needed.
19. Work cooperatively with clients, families, guardians and other stakeholders to ensure client satisfaction with habilitation goals/objectives. Participate as a member of the client interdisciplinary team.
20. Perform other duties as assigned.

Skills

Required Knowledge, Skills, and Abilities

Because of the essential duties and responsibilities of the Habilitation Manager, five years’ experience working with persons with developmental disabilities and supervisory experience is preferred. Knowledge of curriculum development and assessments is preferred. Have self-motivation to research and develop new program ideas and activities to best meet the needs of our diverse population.
In addition, you must: 1) be able to lift up to 70 lbs., walk, bend, crouch, kneel, twist, push, pull, squat, turn in the knees, back, wrists and shoulders and grasp with both hand (in order to properly perform the techniques taught in CPR/First Aid, Client Intervention Techniques and Lifting and Carrying Techniques). 2) Must have normal hearing, with the aid of corrective devices, as needed (i.e., hearing aid). Must have normal vision, with the aid of corrective devices, as needed (i.e., glasses or contacts). 3) Must be free from fainting spells, blackouts, dizziness, or seizures. 4)Must have the ability to sit in a normal seated position for extended periods. 5) Must have the ability to endure extended periods of driving. 6) Must have the ability to tolerate exposure to dust, gas, and/or fumes. 7) Must have the ability to exert up to 20-50 pounds of force, occasionally, and/or 10-26 pounds of force frequently. 8) Must be able to speak, read and write in English. 9) Must be able to complete the Peak Community Services training programs and successfully pass associated tests to meet competency requirements. 10) Must have basic computer skills including, but not limited to, sending and receiving e-mail, accessing web page, entering basic data, use of the Time Keeping System, and common use applications. 11) must have a high school diploma or GED. 12) have a valid Indiana driver’s license and auto insurance. 13) be able to complete all required paperwork and submit by required deadlines and timelines. 14) monitor production and quality control. 15) Some positions require that employees must be able to pass a TB test. 16) Must be able to carry out other duties and responsibilities as assigned.
