Who is Eligible for Our Community Living Homes?
Our community living homes are designed for adults with disabilities who want to live more independently while still receiving the support they need. Residents typically meet the following criteria:

- Are 18 or older and eligible for Indiana’s Medicaid Waiver program
- Have an intellectual or developmental disability
- Need a structured, supportive living environment with access to trained staff
- Are transitioning from another group home or seeking a better fit
- Are referred by a case manager through the Bureau of Developmental Disabilities Services (BDDS)
How to Get Started

3. Complete the Referral & Placement Process – Once your referral is submitted, we’ll help coordinate the move-in process and ensure a smooth transition.
